The images relate to a major health care scandal in which nurse Vanessa Sivadge alleged that Texas Children's Hospital illegally billed Medicaid for transgender procedures. Medicaid is not allowed to be used in so-called “gender reassertion” treatment.

Savidge, who worked extensively with the hospital's transgender patients, realized that it was participate unwittingly in a secret “gender-affirmation” clinic despite the hospital's previous public promises to close it.

A City Journal report in May 2023 detailed how an anonymous whistleblower, now known as Savidge, provided documents about the secret continuation of the program, which included providing young children with implantable puberty blockers.
